What is an occupational health examination?

The Purpose

Protect employees from work-related illness. Early detection of health risks, individual consultation — and legal certainty for employers.

The Legal Basis

Governed by the German Ordinance on Occupational Health Care (ArbMedVV) in conjunction with the Occupational Safety Act (ArbSchG). The employer is responsible — the examination is performed by an occupational health physician.

In Practice

Most examinations follow the DGUV standards (G-examinations). At Samedos in Bremen we perform all standard examinations — at our practice in Bremen-Findorff or directly on-site at your company.

Three examination types

Mandatory, Offered and Requested examinations — what's the difference?

The German ArbMedVV regulation distinguishes three categories of occupational health examinations — depending on who initiates them and whether they are required by law. Correctly classifying them is decisive for your compliance.

Mandatory Examination

Required by the employer for certain activities with specific health risks. Without a current mandatory examination, the activity may not be performed.

Examples:

  • Working with asbestos, lead, mercury
  • Activities with infection risk (TRBA)
  • Noise exposure above trigger thresholds
  • Welding work in confined spaces

Offered Examination

To be offered by the employer for specific activities — participation is voluntary for employees, but the offer must be documented.

Examples:

  • Display screen work (no longer mandatory since 2013)
  • Activities with musculoskeletal load

  • Wet work

Requested Examination

At the employee's request — must be enabled by the employer when a health risk from the activity cannot be ruled out. Despite the "request" nature, it is an employer obligation.

Examples:

  • Employees who feel burdened by their activity
  • For perceived risks without formal threshold breach

Driving, Operating and Surveillance Activities

Work at Heights with Risk of Falling

For employees working at heights above 3 metres — scaffolders, roofers, industrial climbers, offshore personnel. Very strict criteria, because failure is life-threatening.

Examination scope:

  • Detailed physical examination
  • ECG, blood pressure, lung function
  • Balance and vertigo tests
  • Vision test, hearing test
G42 Bremen

Activities with Infection Risk

Mandatory examination for employees with contact to pathogens — nursing staff, laboratory personnel, cleaners with risk exposure, veterinarians, research staff. Often combined with vaccination offers.

Examination scope:

  • Medical history, physical examination
  • Vaccination status check, vaccinations if needed
  • Consultation on protective measures
  • Documentation of pathogen exposure

Musculoskeletal System Stress

Offered examination for employees with physically demanding work — construction trades, warehouse logistics, nursing, production. Aim: early detection of wear-related conditions.

How an occupational health examination works

    Phase 1 — Schedule appointment

    By phone or contact form. We clarify in advance what type of examination is needed and what documents to bring.

  2. 02 30–90 minutes per G-code

    Phase 2 — Examination

    At our practice in Bremen-Findorff or on-site at your company. Medical history, physical examination, instrumental diagnostics if needed.

  3. 03 same day

    Phase 3 — Certificate & documentation

    Certificate for the occupational health records, entry into electronic documentation, possible feedback to you as employer on recommended measures — GDPR-compliant and without medical details.

Examinations directly at your location

For larger employee numbers or logistically complex deployments, we come directly to you — on construction sites, in ports, in industrial halls. On-site appointments save time, reduce downtime and travel, and are usually much appreciated by employees.

Frequently asked questions about occupational health examinations

Are occupational health examinations mandatory for my company?
It depends on the activities of your employees. Mandatory examinations under § 4 ArbMedVV are required for certain activities with specific health risks (e.g., asbestos exposure, noise above trigger levels, infection risk). Offered examinations under § 5 ArbMedVV must be made available for further activities — even if employees decline, you must document the offer. In an initial consultation we clarify what applies to your company.
What does a G-examination cost at Samedos?
Costs vary by G-code and scope. A G25 examination typically costs between €80 and €120, a G41 with detailed physical examination €150–200. A full overview is available in our price list — for larger volumes we offer package pricing.
Where does the examination take place?
Either at our practice in Bremen-Findorff or directly on-site at your company. For individual appointments or smaller groups, the practice is more efficient. For larger employee numbers or specific work environments (construction site, port, wind energy), we are happy to come to you — just get in touch.
How long does a G-examination take?
Between 20 and 90 minutes depending on the G-code. A G37 display screen examination is usually done in 20–30 minutes; a G41 with detailed physical examination can take 60–90 minutes. We plan accordingly and tell you in advance what to expect.
What do employees need to bring?
Valid ID, glasses or contact lenses (if worn), the previous occupational health certificate (if applicable), a list of current medications, and any recent specialist reports. For radiation protection examinations: the radiation passport. For follow-up examinations: the previous occupational health record certificate.
Who covers the cost of occupational health examinations?
The cost of occupational health examinations under ArbMedVV is borne entirely by the employer. Examinations take place during working hours and count as working time. We bill directly with your company — employees are not involved in invoicing or payment.
